Our team provides a range of services to ensure effective and safe care for older adults. The team encompasses Case Management, Service Coordination, Administration and Direct Care Supports, aiming to optimise the health and quality of life of clients through coordinated, person‐centred, supportive care.
Job Description
Location: Eastern, Outer Eastern and Yarra Valley regions (office based in Ferntree Gully). Hybrid work model (home visits + office + WFH where approved). Part‐time (0.8 FTE) ongoing position – days negotiable.
Compensation: $44.78 – 48.99 per hour + Not‐for‐Profit Salary Packaging to maximise take‐home pay.
The Role
As a Care Partner you will provide older adults, their families and carers with access to responsive care and case management, ensuring services and support are tailored to each individual's needs and requirements. Typical day includes liaising with older persons via phone or home visits, following up on care requirements and supporting their goals for happy, fulfilled lives while remaining independent.
Responsibilities
* Utilise case management skills to undertake initial and ongoing comprehensive assessments and care planning for clients at home.
* Manage delivery of services to support identified needs (psychosocial, medical, physical, vulnerabilities) within the customer's individualized budget.
* Build and maintain relationships with colleagues, service networks and community agencies to support the program and customers.
Qualifications and Experience
* Australian Drivers Licence.
* Minimum 2 years' experience in case management in home and community settings.
* Qualifications in Aged Care, Health, Community Services or related discipline (highly desirable).
* Experience building professional relationships with customers, supporters and external service networks.
* Understanding of My Aged Care system.
* Alignment with Each's values and principles for care of older adults.
* Knowledge of Aged Care Quality Standards, Community Home Care / Support at Home.
