About this Role
This position is focused on assisting families, caregivers, and healthcare professionals in understanding how dementia affects each individual.
We are seeking an experienced professional to synthesise information, use observation and documentation, and provide recommendations based on evidence-based best practices.
The ideal candidate will be able to contextualise reports for specific situations and people, resulting in positive change on the ground and improved quality of life for those living with dementia.
Key Responsibilities
* Support carers and clinicians to prevent and respond to changes in behaviour through coaching, resource development, and modelling.
* Develop behaviour support plans that cater to individual needs.
* Conduct a range of assessments to understand the holistic experience of individuals and their care networks.
* Work collaboratively with Dementia Support Australia to build capability across the residential aged care and community sectors.
* Contribute to research, projects, and programs, using innovative methods to meet changing sector needs.
* Other tasks as required, aligning with areas of skill, expertise, and knowledge.
Essential Skills
* Expertise in supporting individuals with dementia.
* Experience in Australian residential aged care or community aged care.
* Strong communication, negotiation, and relationship-building skills.
* Excellent problem-solving, decision-making, and time management abilities.
* Current Australian driver's licence and willingness to travel.
* Relevant qualifications in Allied Health, Nursing, Aged Care, or Mental Health.
