**About the role**
This position is within the dynamic, multidisciplinary Community Rehabilitation Team and involves the provision of clinical case management services to complex clients including those with an acquired brain injury and complex neurological conditions.
**Key Responsibilities**
- Comprehensive assessments and plans
- Assisting clients to develop and met their rehabilitation goals
- Coordination of services and referrals for the complex client
**Skills and experience**
This position would suit a Social Worker or Occupational Therapist who has knowledge of neurological conditions and the impacts on the rehabilitation process and who is looking to develop an additional skills in supporting clients through the rehab process.

**Selection Criteria**
- Essential : Eligibility for practicing membership of AASW or current APHRA registration.
- Desirable : A sound knowledge of and experience in case management, and the comorbidities that commonly occur with ABI and other neurological conditions.
- Essential : Demonstrated ability to develop and empower others through supervision, modelling, and training.
- Essential : Demonstrated ability to identify, and actively participate in, quality improvement activities and positively influence change as required.
- Essential : Current drivers' licence
