About The Opportunity

Are you a dedicated Social Worker eager to improve end-of-life care within the community? Become a vital part of our multi-disciplinary team and positively impact the lives of patients, caregivers, and families navigating life-limiting illnesses during their last year. Help us deliver compassionate, person-centred support that enhances quality of life when it matters most.

As an end-of-life care Social Worker, you will provide multidisciplinary support to clients and carers referred to the community based End of Life Care service within SWSLHD, always upholding professional codes of ethics and the NSW Health Code of Conduct. You will manage a clinical caseload, conducting thorough psychosocial assessments within clients' homes. Collaborating closely with clients, families and carers, and the multidisciplinary team, you will develop, implement, and review individualised care plans that address the unique emotional, social, and practical needs of those facing life-limiting illnesses, ensuring compassionate, person-centred support throughout the end-of-life journey

What You'll Be Doing

To provide Social Work services to consumers and their carers to ensure the provision of optimal health outcomes consistent with SWSLHD policies and procedures, standards, relevant legislation, and the NSW Health Code of Conduct. The position will also participate in professional development, clinical supervision, quality activities and other relevant departmental and administrative duties.

Where You'll Be Working

SWSLHD Primary & Community Health provides a comprehensive range of community-based services designed to improve and maintain the health and wellbeing of individuals and communities across South Western Sydney.

These services include prevention, early intervention, assessment, acute and post-acute treatment, health maintenance, and continuing care. The team supports diverse populations, offering specialised programs for children, young people, older adults, and at-risk communities.

With over 800 dedicated staff, including Aboriginal Health Workers, nurses, doctors, and allied health professionals, the service ensures high-quality care close to where people live.

How To Apply

* Degree in Social Work which provides eligibility for membership of the Australian Association of Social Workers.
* Demonstrated experience in a health related field and ability to provide a range of Social Work interventions.
* Ability to work independently at a professional level as well as the ability to work collaboratively as a member of a multidisciplinary team in a community health setting.
* Evidence of commitment to clinical supervision and professional learning.
* Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
