A bit about the role
Reporting to the Service Manager this role is responsible for ensuring quality care and support is delivered in a customer-centric way to enable more older Australians to remain living at home safely. The role is to support clients living in the Logan, Southern Brisbane region.
Hybrid flexible working arrangements – Monday to Friday, full-time.
You will be responsible for the planning and oversight of care and supports that meet client goals which deliver exceptional service.

Key responsibilities
•Planning and oversight of the scheduling of support staff and third-party suppliers to deliver quality care and services. Collaboration with clinically qualified Care Partners such as Nurses and Allied Health Practitioners
•Working collaboratively with the Service Manager to undertake staff orientations, education, staff performance appraisals and performance management.
•Responsible for the development and review of Care Plans in consultation with clients and their families and pathway-specific coordination
•Ensure individual clients' budgets are accurately recorded and monitored regularly to reflect the care plan
•Regularly reviewing and updating client Care Plans, identifying risk, documentation and compliance particularly relating to clinical governance (Aged Care Standards).
Key selection criteria
•12 months experience ideally in aged support services
•Highly professional and able to act with compassion
•Exceptional time management skills
•Excellent administrative and computer skills (Google, MS Office)
•Excellent communication skills, both oral and written
Requirements
•Social, Home & Community Services, Aged Care, Disability, Health Industry or related relevant experience
•Nursing qualification is preferred but not essential
•Current Australian driver's licence and safe driving record
•Own car & willingness to travel to visit customers
