The Care Manager will be responsible for ensuring that clients receive seamless service delivery through close collaboration with our office team.
Key Responsibilities:
1. Building strong relationships with clients, families, caregivers and other stakeholders to identify evolving needs and preferences.
2. Maintaining up-to-date knowledge of community services, health providers and referral sources to promote available support options.
3. Collaborating with clients and their representatives to ensure care plans remain aligned with changing needs and preferences.
4. Conducting regular quality assurance visits to monitor care, address any concerns and recommend additional services as required.
Your Skills and Qualifications
* Tertiary qualification in a relevant field (e.g., social work, allied health, community services) is desirable.
* Proven experience in Aged Care or Disability is essential.
* Excellent written and verbal communication skills, with accurate data entry and proficiency in Microsoft Office.
* A valid Australian Driver's License is also required.
