Job Title: Specialist Disability Support Practitioner
This role is a 12-month fixed term contract, providing case-managed assistance to children and parents with disabilities. The key focus is on supporting vulnerable families to access and actively engage with disability supports and skill development opportunities.
About the Role:
* Support families in accessing and participating in the NDIS.
* Assist families in navigating systems of disability support, including the NDIS, disability advocates, service providers, and mainstream services.
* Provide disability-related case consultations to the broader Child and Family Services system within the Mallee area.
The successful candidate will work within the context of the Mallee Child and Family Services Alliance, which is an alliance of seven agencies. Most referrals for this service are sourced through the Orange Door, which is the central intake point for families needing assistance with the care and wellbeing of children in the Mallee.
Key Responsibilities:
1. Case management and coordination of support services for families with disabilities.
2. Development and implementation of individualized plans to support families' disability needs.
3. Collaboration with other service providers and stakeholders to ensure a holistic approach to supporting families.
