Opportunity for a suitably experienced Support Worker
Based in Mandurah and surrounding areas
Part-time role – $***** per hour (Level 2) or $***** per hour (Level 3) + Salary Packaging options and Superannuation (inclusive of casual loading)

Our Home Care services (CHSP, VHC and HC) empower older people and veterans to live safely whilst enjoying as much independence as possible within their own homes. You will provide individualised care tailored to the needs of each client and their families.
Monday to Friday – 8.30am to 2pm – Permanent hours
Primary responsibilities:
Meal assistance, including shopping and meal preparation
Transport and accompaniment, including shopping trips and appointments
Social support & activities, including outings and social activities
Personal care, including assistance with personal grooming, hygiene, bathing, dressing
Provide support for health conditions, including skin care, medication reminders and diabetes management
Assistance with mobility
Domestic assistance, including cleaning and laundry
What you will need:
A kind, caring and respectful approach to service provision
Certificate III in Aged Care / Individual Support (or similar) or equivalent experience
Self-motivated with the ability to work effectively in both a team environment and independently with minimal supervision
Ability to work with individuals from a range of social, financial and cultural backgrounds
Strong communication and interpersonal skills
Current driver's license and reliable car
Current national police check (not older than 6 months)
Prior experience in a similar role in the community/home care (desirable) or Disability/Residential care
