Job Overview:
We are looking for a passionate and dedicated NDIS Support Coordinator and Psychosocial Recovery Coach to join our team.
In this role, you will assist NDIS participants with varying abilities in achieving their NDIS plan goals, enhancing their daily living skills, and promoting their overall well-being.
You will report directly to the Capacity Building Team Lead, who is committed to supporting your professional growth and success.

Key Responsibilities of this role include but not limited to
NDIS Support Coordination:
Assist NDIS participants in understanding and implementing their NDIS plans.
Collaborate with participants to identify goals and supports needed to achieve them.
Liaise with service providers and coordinate the delivery of NDIS-funded services.
Maintain accurate records and documentation of participant progress and services provided.
Psychosocial Recovery Coaching:
Provide one-on-one psychosocial recovery coaching to help participants build resilience and improve mental health.
Develop recovery plans and strategies tailored to each participant's unique needs.
Offer emotional support, and skill-building to enhance coping and self-management.
Monitor and evaluate the effectiveness of coaching interventions and adjust plans as needed.
To be successful in this position you will have -
1-2 years' experience in direct case management and extensive knowledge in the NDIS or related field.
Background in Social Work, Psychology, Community Services, or similar fields preferred but not mandatory.
NDIS Worker Screening Check (required prior to the start of employment).
WA Driver's license and Licensed and Insured vehicle
Current and valid Working with Children Check
Current and valid First Aid & CPR certificates
