In this pivotal role, you will coordinate safe, timely, and effective discharge planning for patients transitioning from hospital care to home or community services. You will work collaboratively with medical, nursing, and allied health teams to ensure continuity of care and optimise patient outcomes, while supporting families and carers through the discharge process. * To provide expert clinical and patient-centred care within the Palliative Acute Rehabilitation, Acute Stroke Unit (PARAS) by supporting the facilitation and provision of quality and cost effective nursing services which are consistent with contemporary standards and evidence-informed practice.

Our offer: You will receive a competitive remuneration package value up to $131,900 p.a., comprising salary between $107,960 - $115,604 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (NRG6(1)) (Permanent Full Time, 76 hrs p.f.)
