About the role:
Reporting to the Head of Community Care and Home Care Services, the Clinical Lead is a senior clinical position responsible for:
* Providing leadership and clinical oversight to a multidisciplinary team of Care Managers (including RNs, ENs, and non-clinicians)
* Driving the delivery of safe, effective, and person-centred care across all community programs
* Supporting complex client care planning, reassessment, and clinical escalation processes
* Monitoring and responding to clinical risks, incidents, and compliance requirements
* Ensuring adherence to the Aged Care Quality Standards, the Aged Care Act, and the Support at Home reforms (commencing November 2025)
* Supporting onboarding, mentoring, and professional development of frontline care staff
* Acting as a key clinical interface with external providers, allied health, and regulatory bodies
What we need from you first:
* AHPRA registration, post graduate preferred
* Current Flu vaccination evidence
* COVID-19 Australian Government vaccination certificate
* Current National Police Check (not older than 18 months)
* NDIS Clearance
* Current driver's license/reliable source of transport
* Wonderful people skills- can build rapport easily with facilities and their residents while representing CBH
