About the Role

We're seeking a compassionate, experienced and confident Support Coordinator / Specialist Support Coordinator who values connection, enjoys working as part of a team, and is passionate about supporting individuals, families and communities.

In this role, you'll work alongside NDIS participants with complex needs to implement their plans, build capacity, and navigate multiple service systems. You'll support participants experiencing significant barriers, including those involving risk, crisis, housing, mental health, justice and complex family dynamics.

You'll take a lead role in coordinating supports, managing risk and working collaboratively with a wide range of stakeholders, including families, service providers, health services and community organisations, to ensure supports are safe, effective and aligned with participant goals.

This role requires strong professional judgement, problem‐solving and the ability to work flexibly and independently, while remaining connected to a supportive team environment.

You'll have the opportunity to be involved in outreach across the Kimberley, including travel to remote communities, with allowances provided. You'll contribute to a flexible, person‐centred approach that supports meaningful outcomes for participants and their communities.

There is an opportunity for the right candidate to advance into senior positions within the Support Coordination (SC) team if prior leadership or management experience is demonstrated during the probationary period.

Key Responsibilities
* Support participants to understand and implement their NDIS plans.
* Coordinate services and supports across health, disability, housing, justice and community sectors.
* Work collaboratively with participants, families and service providers to achieve goals.
* Advocate for participants and support them to overcome systemic and personal barriers.
* Identify, assess and respond to risk, including crisis situations; develop and implement risk‐management and support strategies.
* Monitor supports and adjust arrangements in response to changing needs.
* Maintain accurate, timely and compliant documentation.
* Contribute to a flexible service model, including community‐based work and outreach where appropriate.
* Participate in team meetings, supervision and ongoing professional development.
About the Practice

Kimberley Therapy Services is an ever‐growing, locally owned multidisciplinary allied health and support services provider delivering services across the Kimberley. We are dedicated to supporting individuals and families to build capacity, strengthen independence and create meaningful connections within their communities.

Our team includes Speech Pathologists, Occupational Therapists, Physiotherapists and Support Coordinators working together to deliver holistic, person‐centred care. We are committed to inclusive, collaborative and evidence‐informed practice, recognising the importance of dignity of risk and that the people we support are the experts in their own lives.

We are based in the Kimberley region of Western Australia, with offices in Broome, Derby and Kununurra. Our services are delivered in clinics, homes and schools, along with regular outreach across the Kimberley to communities such as Fitzroy Crossing, Halls Creek, Warmun, Bidyadanga and Looma.

What We Are Looking For
* A minimum of six months of experience in support coordination, case management, community services or a related field. Social work or clinical experience is also accepted.
* A strong understanding of the National Disability Insurance Scheme (NDIS) or a willingness to learn about it.
* A positive, team‐oriented approach along with strong communication skills.
* A genuine interest in working with individuals, families, and communities.
* A commitment to delivering culturally safe and person‐centred services.
* Strong organisational and time‐management skills.
* An openness to learning and developing your skills.
* A relevant tertiary qualification in Social Work, Occupational Therapy, Nursing or a related discipline, or demonstrated extensive experience working within the NDIS supporting participants with complex needs.
* Demonstrated experience working with participants with complex needs, including risk and crisis situations.
* Advanced problem‐solving and decision‐making skills.
* An interest in rural and remote practice.
