About the opportunity
* Responsible for conducting client assessments, accessing the respite services, service development and rostering set up of clients.
* Support growth of the program, building on the current CHSP program along with growth in private client case loads
* Review procedures, administration, reporting requirements and community with program stakeholders
* Be part of a multi-disciplinary team which requires you to provide highly responsive and flexible service to consumers and carers

To thrive in this role, you will have:
* Relevant community care experience in intake, assessment and care planning
* Excellent knowledge of CHSP, NDIS, VHC, DVA, HCPs
* Excellent, time management, organizational and communication skills, with the ability to adapt and problem solve
* Understanding of the needs of people from special needs groups
* The ability to nurture friendly, respectful relationships and contribute to a collaborative team environment.
* Experience with Microsoft office suite
