About the role
:
1. Provide holistic and culturally appropriate palliative care and end of life services to Aboriginal patients and their families and carers.
2. Work closely with the palliative care multi-disciplinary team, community based service providers, patients and their families to ensure cultural and community issues are adequately considered.
3. A comprehensive orientation package will be set up for the successful applicant as well as ongoing professional development opportunities including, mandatory training, regular clinical supervision, peer support as well as support from Social Work Manager.
4. Work within a multidisciplinary team environment and contribute to the social work perspective in the multidisciplinary response, ensuring that patients are supported through the health system and beyond their discharge from the hospital.
Requirements:
5. Hold AHPRA registration as an Aboriginal Health Practitioner, or willingness to obtain AHPRA registration as an Aboriginal Health Practitioner.
6. Holds or aspires to hold a minimum Certificate III qualification in Aboriginal Primary Health Care or a minimum Certificate III health qualification in the area of care in which the Aboriginal Health Worker works.
7. Eligible to drive in NSW and the ability to travel as required.
8. Holds or aspires to hold a minimum Certificate III qualification in Fitness and including the Group Fitness module.
