Our services operate throughout Greater Sydney, year-round, to help people achieve their goals and enhance their health and wellbeing.
**What We're Looking For**:

- **Person-Centred**: Liaise with healthcare professionals, families, and clients to deliver high-quality, person-focused services.
**Key Responsibilities**:
- Coordinate individualised health and wellbeing plans that align with each person's goals
- Act as a liaison between clients, healthcare professionals, and family members
- Provide effective supervision and leadership to support workers, ensuring high standards in service delivery
- Ensure compliance with WHS, NDIS, and organisational policies
- Regularly visit people CCA supports to monitor service effectiveness, identify areas for improvement, and implement Health & Wellbeing action plans
**Qualifications**:
- Diploma of Nursing/Certificate IV in Nursing (Enrolled Nurse) or Certificate IV in Disability Services
- Experience in developing personalised care plans and working with individuals with a range of health care needs
- Proven experience managing and mentoring support staff
- Strong understanding of WHS, legal, and regulatory requirements related to health care and case management
- Current driver's licence and NDIS Quality and Safeguards Worker Screening
