The heart of the role
As Care Manager you will work as part of a team of empathetic and caring clinical professionals. You will support the General Manager in managing and leading the delivery of person-centred care for residents, ensuring the highest standard of ongoing assessment, care planning and evaluation, and clinical governance—helping them to live longer, healthier, and happier lives.

Duties include
* Oversee the daily clinical operations of the care home
* Support and lead the care home clinical team including coaching, mentoring, staff allocations and delegation, performance management and education
* Review clinical incidents, complaints and changes
* Lead compliance with AN-ACC assessments
* Manage person-centred care planning and evaluation for residents, including case conferencing with families and carers
* Prepare and conduct clinical governance audits
* Oversee medication management in accordance with resident needs and legislative requirements
* Liaise with allied health, medical practitioners and other services to provide optimal resident care

What will you bring?
* Bachelor of Nursing with current AHPRA registration (essential)
* Experience managing a minimum of 30 beds
* Proven track record in delivering quality clinical care within an aged care or acute setting
* Excellent clinical assessment skills and care planning capabilities
* Highly effective communication, leadership, and relationship management skills
* Understanding of Resident Rights, Aged Care Accreditation Standards and Outcomes
