Job Description
We are seeking a dedicated and compassionate Nurse to join our team in providing high-quality community-based nursing care to clients in their homes.
The successful candidate will work independently and as part of a multidisciplinary team, assessing, educating, intervening, and developing discharge plans for clients accessing our Transitional Aged Care Program.
* Strong clinical judgment and problem-solving skills
* Ability to provide safe, compassionate, quality care that is culturally responsive
* Effective communication and engagement with patients, carers, and the multidisciplinary healthcare team
