Key Responsibilities
1. Participant Support
2. Provide direct support to participants in areas including personal care, meal preparation, medication administration, and household tasks.
3. Assist participants in accessing the community, attending appointments, and engaging in social, recreational, and employment activities.
4. Promote participant independence and capacity building through skill development.
5. Person-Centered Approach
6. Develop and maintain respectful and professional relationships with participants and their families.
7. Follow individual support plans, behavior management plans, and goals outlined in NDIS plans.
8. Monitor and report changes in participants' needs or circumstances.
9. Compliance and Reporting
10. Document daily activities, progress notes, and incidents in line with organizational policies and procedures.
11. Maintain confidentiality and adhere to the NDIS Code of Conduct and Safeguarding requirements.
12. Support participants in managing risks and ensure a safe environment.
13. Team Collaboration
14. Work collaboratively with the Team Leader and other support workers to provide consistent and reliable services.
15. Attend team meetings, training sessions, and professional development activities.
16. Communicate effectively with team members regarding participant progress and any challenges faced.
