Mix of company car use and use of own car with mileage reimbursement The Role As the Clinical Lead you will: - Hold a caseload of 6 aged community clients with complex clinical and psychosocial needs Provide advanced clinical oversight and care planning Review and update care plans across the broader service (approx. 50 clients) Work closely with and report to the Home Care Manager Support care delivery for clients with multiple comorbidities, cognitive and behavioural considerations, and diverse living environments, requiring adaptability and a holistic approach What You'll Bring Current AHPRA registration as a Registered Nurse Experience managing complex cases, ideally within community, mental health, or vulnerable populations Strong skills in clinical assessment, care planning, and coordination Confidence working autonomously while contributing to a collaborative team A genuine passion for equitable healthcare and social impact Aged Care experience is ideal, with knowledge of the Support at Home program Why apply through Healthcare Australia?
