The Position As a Home Care Physiotherapist, you’ll provide tailored, one-on-one care in clients’ homes across the Hobart region. You’ll manage a diverse caseload that may include NDIS, DVA, and Home Care Package clients. All work-related tasks like programming, reporting, and meetings are included in your paid hours. Your responsibilities will include : Provide physiotherapy services to clients in their homes, ensuring person-centred, goal-oriented care Conduct assessments, develop treatment plans, and deliver rehabilitation tailored to each client’s needs Work closely with families, carers, and other health professionals to ensure optimal client outcomes Contribute to a positive team culture that supports ongoing learning and peer support What we’re looking for: Tertiary qualification in Physiotherapy Current full AHPRA registration (or working towards) Current Australian driver’s license CPR & First Aid certificate A passion for community health and working one-on-one with clients in their own environment Strong communication skills and a collaborative mindset Current driver’s licence and access to a reliable vehicle Experience in aged care, rehabilitation, or community-based services is highly valued Ability to maintain complete and accurate clinical documentation The Organisation Leap Health is a trusted name in allied health across Tasmania, known for their quality care, strong client relationships, and supportive team culture.
