We're seeking a passionate Senior Rehabilitation Support Worker to join our new Modbury Mental Health Rehabilitation Unit, supporting individuals and families on their journey to wellness. About the role: The Modbury Mental Health Rehabilitation Unit delivers high-quality rehabilitation services through a multidisciplinary team committed to person-centred, trauma-informed care. Our collaborative environment focuses on compassionate, solution-focused approaches that promote skill development, social inclusion, and empower consumers to achieve their recovery goals. You'll work closely with Wondakka Community Rehabilitation Centre and The Gully services, contributing to mental health rehabilitation innovation while fostering continuous improvement and co-design practices. As the Senior Rehabilitation Support Worker, you'll provide expert rehabilitation support to consumers experiencing mental health challenges. Working within our collaborative multidisciplinary team, you'll facilitate recovery-focused programs, support daily living skills development, and empower individuals on their journey toward independence and wellbeing. In addition, you will deliver person-centred rehabilitation interventions and group programs, assess client needs and develop individualised support plans, mentor junior staff and contribute to service improvement initiatives, collaborate with psychiatrists, nurses, occupational therapists, and social workers, and maintain accurate documentation while participating in case reviews.
