We are seeking a Case Manager to join our Housing Mental Health Pathways Program.


About the Opportunity

The Case Manager will play a crucial role in preventing individuals from being discharged from acute psychiatric inpatient stays into homelessness by providing specialized housing interventions at the time of discharge. The program is a partnership between Launch Housing and acute mental health programs at local hospitals.

The position involves engaging with individuals experiencing or at risk of homelessness while they are an inpatient at the psychiatric unit, working collaboratively with patients, hospital staff, and community supports to develop a comprehensive housing support plan. This may involve ongoing support after discharge until clients have secured safe and sustainable housing.


Key Responsibilities

* Undertake housing-focused assessments and advocacy work with individuals during their admission in the acute mental health program.
* Collaborate with clients, hospital staff, community mental health services, Specialist Homelessness Services, and other relevant agencies to address the housing support needs of inpatients who are homeless or at risk of homelessness.
* E nsure clients are linked with appropriate housing and support following discharge from hospital.
* Assess clients on the ward or where they are located post-discharge, offering housing assistance and referrals to local agencies as necessary.
* Attend clinical reviews for inpatients of the psychiatric inpatient unit as required.


Requirements

* A tertiary qualification in social work, nursing, psychology, occupational therapy, or another relevant allied health discipline.
* An understanding of the mental health service system, including clinical and non-clinical services.
* Demonstrated ability to build positive relationships and communicate with people of diverse backgrounds/abilities.
* Good interpersonal and communication skills, both verbal and written.
* Well-developed organizational, task prioritization, and time management skills.
