We are seeking an experienced Care Manager to join our team at a leading aged care and dementia provider. As a key member of our clinical leadership team, you will play a vital role in delivering high-quality person-centred care to our residents.
The Role:
As a Care Manager, you will be responsible for managing the daily clinical operations of our care home, ensuring the highest standard of ongoing assessment, care planning, and evaluation. You will work closely with our General Manager to lead the delivery of person-centred care, coaching, mentoring, and supporting our clinical team to achieve exceptional results.
Key Responsibilities:
* Oversee the daily clinical operations of the care home
* Support and lead the care home clinical team, including coaching, mentoring, staff allocations, and delegation
* Review clinical incidents, complaints, and changes
* Lead compliance with AN ACC assessments
* Manage person-centred care planning and evaluation for residents, including case conferencing with families and carers
* Prepare and conduct clinical governance audits
* Liaise with allied health, medical practitioners, and other services to provide optimal resident care
Requirements:
To be successful in this role, you will require:
* A Bachelor of Nursing degree with current registration
* Post-graduate qualification in aged care (highly regarded)
* Experience managing a minimum of 30 beds
* Proven track record in delivering quality clinical care within an aged care or acute setting
* Excellent clinical assessment skills and care planning capabilities
* Highly effective communication, leadership, and relationship management skills
* An understanding of Resident Rights, Aged Care Accreditation Standards, and Outcomes
