About the Role
Juniper is seeking a culturally responsive and community-focused Care Partner to join our Home and Community Care (HCC) team in the Kimberley region. In this vital role, you'll be the trusted connection for our Aboriginal and Torres Strait Islander customers and their families, building strong relationships, coordinating services, and ensuring care is delivered with respect, empathy, and cultural understanding.
You'll oversee a portfolio of government-funded programs including Support at Home (SAH), Commonwealth Home Support Program (CHSP), and the National Aboriginal and Torres Strait Islander Flexible Aged Care Program (NATSIFAC). Your leadership will ensure services meet the highest standards of care and compliance under the Aged Care Act 2024 and the Strengthened Aged Care Quality Standards.

Key Responsibilities:
* Manage and coordinate community-based aged care programs with a focus on customer goals and wellbeing
* Lead and support Community Support Workers through recruitment, onboarding, supervision, and development
* Monitor client health and wellbeing, ensuring early intervention and appropriate referrals
* Deliver trauma-aware, healing-informed care that respects each individual's story
* Ensure compliance with legislation, funding guidelines, and Juniper's policies
* Promote a safe and healthy work environment through proactive WHS practices
What You'll Bring:
* Experience in case management or coordination within aged care, disability, or community services
* Strong understanding of aged care funding programs and person-centred care principles
* Excellent communication and leadership skills
* Ability to manage budgets, develop care plans, and support staff performance
* Relevant qualifications in community services, aged care, or health (minimum Certificate IV)
* Current driver's licence and access to a reliable vehicle
* National Police Clearance
