Clinical Nurse Specialist - Community Health Opportunity

The WA Country Health Service is seeking a talented and enthusiastic Clinical Nurse Specialist to join the Community Health Team in the Midwest. This role involves coordinating primary health care for vulnerable children and families within the community setting.

Key Responsibilities:

* Coordinate primary health care services for vulnerable children and families in the community.
* Collaborate with key stakeholders, including healthcare professionals and community organizations, to improve health outcomes.
* Collect and analyze data to inform program development and service delivery.
* Act as a role model and consultant to other health professionals, providing guidance and support on community health nursing practices.

Required Skills and Qualifications:

* Significant clinical knowledge, experience, and leadership in community health nursing.
* Knowledge of cultural factors and social determinants, particularly relating to Aboriginal health.
* Advanced interpersonal and communication skills in a multidisciplinary team.
