Join our Central Adelaide palliative care team and provide vital psychosocial support to patients and families. You will manage complex cases, build care plans, create continuity across services and maintain strong collaborative relationships with teams and external agencies.
Duties
* Deliver psychosocial assessments and interventions for patients and families
* Provide counselling, bereavement support and specialist advice
* Collaborate with medical, nursing and volunteer teams across the Central Adelaide Palliative Care Service (CAPCS)
* Guide service planning, review care models and share best practice
* Liaise with external agencies and support continuity across multiple sites
About You
You are a compassionate Social Worker ready to support patients and families through complex palliative care needs with skill and empathy.
* Relevant degree with eligibility for full membership of Australian Association of Social Workers
* Deliver expert psychosocial assessments and counselling
* Work effectively in interdisciplinary teams across settings
* Provide family support and guidance for complex situations
* Apply social work programs, policies and evidence‐based practice
