At UnitingSA, we're looking forpassionate and experienced Registered Nurses tojoin our Community Aged Care team. What You'll Be Doing

As a Community Registered Nurse,you'll travel across metro Adelaide to deliver exceptionalperson-centred care, that empowers clients to live their best livesand continue thriving in the comfort of their own homes.

With the support of your experienced and friendlyClinical Nurse Specialist, you will;

* Provide hands-on clinical care including dementia care,medication management, wound care and chronic diseasemanagement
* Undertake comprehensive and holistic assessments andcontribute to the client's individual care plan
* Monitoring and documenting clients' health status andprogress, including reporting of signs of clinicaldeterioration of clients
* Support early intervention strategies to reduce hospitaladmissions
* Build strong, supportive relationships and communicateeffectively with clients, families, care providers and healthprofessionals to establish goals, provide support andeducation
* Educate and empower clients to manage their health withconfidence
* Promote dignity, autonomy and choice in everyinteraction
* Work autonomously while staying connected to a supportiveteam of nurses and health professionals
* Ensure compliance with Aged Care Standards and clinicalgovernance requirements.

What You Bring

* Registration with AHPRA as a Registered Nurse
* A Bachelor of Nursing and current First AidCertificate
* Recent nursing experience in aged care, community or homecare settings
* Strong clinical skills in chronic disease management, woundcare and dementia
* Strong clinical knowledge and understanding of contemporarynursing practices
* Excellent interpersonal, communication and time managementskills.
* Ability to work independently and as part of a unitedteam
* A warm, respectful communication style and a deep respectfor older people
* A current SA driver's licence, your own reliablevehicle and comprehensive car insurance
* A current NDIS Worker Screening Check (or willingness toobtain)
* Proficiency with clinical documentation systems
