About the Role
We are seeking a skilled Change Management Coordinator to join our Clinical Risk and Governance team on a permanent, full-time basis.
This role involves delivering, implementing, and embedding strategies and education to ensure the long-term sustainability of care models within our facilities.

Key Responsibilities
* Collaborate with our leadership team, supporting the management of facilities that provide a seamless care continuum for residents and consumers.
* Deliver, implement, and embed strategies and education to ensure the long-term sustainability of care models within our facilities.
* Support the management of facilities through project management and change management expertise.
Requirements
* Unrestricted AHPRA registration as a Registered Nurse.
* Experience in change management, including research and evaluation skills.
* An understanding of primary nursing and team care models.
* Willingness to leverage technology to improve outcomes.
* Experience building stakeholder relationships for collaborative success.
