The Role
We are looking for motivated Registered Nurses with highly developed clinical practice, decision-making, client advocacy and sound assessment skills to join the Community Palliative Care Team. The Community Palliative Care Nurse:
- Provides holistic assessment and care planning to support symptom management and enhance quality of life
- Integrates psychological and spiritual aspects into client care
- Offers support to help clients live as actively as possible
• Offers support to families and carers during the client's illness and throughout bereavement
- Uses a multidisciplinary team approach to address the needs of clients and their families or carers
- Ensures effective communication and coordination with families, carers, and external providers to deliver seamless care.
