Child and Family Health Practitioner
We are seeking an experienced Child and Family Health Practitioner to join our multidisciplinary health care team. As a key member of the Building Strong Foundations program, you will be responsible for providing high-quality services to patients and clients across the Western NSW Local Health District.
About the Role
The successful applicant will have a strong passion for coordinating support for patients and families while they're in our care. You will work collaboratively with other healthcare professionals to provide quality services that meet the health and wellbeing needs of our community.
Key Responsibilities
* Provide culturally safe and appropriate early childhood health services to Aboriginal children and their families.
* Work collaboratively with other healthcare professionals to develop and implement comprehensive care plans.
* Conduct home visits and community outreach programs to support families and promote healthy development.
* Collaborate with local community organizations and service providers to enhance access to services and resources.
Requirements
To be considered for this role, applicants must have:
* A degree in social work or a related field.
* Post-graduate qualifications in child and family health practice.
* Experience working with Aboriginal communities and families.
* Strong communication and interpersonal skills.
