Out of Hospital – Home Care Worker (Griffith)
Community Services #1 is seeking a Casual Out of Hospital Home Care Worker to deliver high-quality, person‐centred care that supports independence and wellbeing.

Community Services #1 is a not-for-profit supporting people and families across the ACT and surrounding NSW. With over 200 employees and volunteers, we deliver services that strengthen community wellbeing and belonging.
About the role:
* Provide home care services aligned to Out of Hospital Care goals, including domestic assistance, personal care, support services that assist recovery and independence.
* Assist participants during the end‐of‐life stages of care.
* Apply duty of care and safe work practices in client homes, and identify, document, and escape OHC escalation triggers promptly.

About you:
* Certificate III in Individual Support (Ageing) or equivalent, relevant experience delivering home care/aged care support (domestic assistance and/or personal care) and 3+ months aged carer experience.
* Experience supporting clients with diverse cultural, emotional and health needs in community or out‐of‐hospital settings.
* Ability to follow care plans and monitor client wellbeing.

Casual role. Remuneration aligned to SCHADS Award – Home Care Employee Level 2 and 3 (casual), plus super. Supportive team, development opportunities and free parking.
Requirements:
Current driver's licence and ability to obtain/maintain required pre‐employment checks and certifications (including National Police Check, Working with Children Check and First Aid), plus mandatory training as required.
