About the Role:A leading community aged care provider is seeking an experienced and compassionate Registered Nurse Case Manager to join their dedicated team in Melbourne. In this vital role, you'll support older Australians by managing a caseload of clients, developing person-centred care plans, and providing holistic support tailored to their medical, emotional, and social needs.As a trusted advisor to clients and their families, you'll work in collaboration with a multidisciplinary team to ensure high-quality, consistent care.
