About the Role:
This is an exciting opportunity to join our team as a Full Time Care Coordinator. As a key link between our clients, their families, and our care team, you will be responsible for ensuring that our clients receive the highest quality home care services.
Main Responsibilities:
* Match clients with caregivers based on their needs and preferences.
* Schedule and coordinate in-home care services.
* Communicate effectively with families, caregivers, and healthcare professionals.
* Monitor and update client care plans to ensure compliance with NDIS standards and aged care best practices.
What We Are Looking For:
We are seeking an experienced Care Coordinator who has a strong understanding of aged care and disability care principles. You will have excellent organizational and communication skills, with the ability to multitask and work in a fast-paced environment. A caring and compassionate attitude is essential for this role.
Requirements:
2 years' experience in aged care/disability care and knowledge of NDIS standards and Aged Care Standards preferred.
Certificate III/IV in Individual Support (Ageing and Disability) or equivalent.
Strong computer skills, including proficiency with scheduling software.
