About The Role
In this position, you will be responsible for the assessment, planning, delivery, and evaluation of person-centred care, ensuring compliance with aged care accreditation standards. You'll lead care teams within your scope of responsibility and collaborate closely with residents, families, and multidisciplinary professionals to achieve the highest quality outcomes.
Key Responsibilities
* Clinical Care:
o Conduct baseline and focused assessments using a Positive Wellbeing Model
o Develop and evaluate evidence-based care plans in consultation with residents and care teams
o Monitor health outcomes and respond promptly to changes in condition
o Accurately document care in accordance with electronic systems and legislation
o Liaise with internal and external healthcare professionals to coordinate care
* Leadership & Quality:
o Provide leadership to care teams, fostering a culture of respect, collaboration, and clinical excellence
o Support workforce management including mentoring staff and students
o Ensure policy and compliance requirements are met, including reporting and quality documentation
o Promote a zero-harm safety culture and drive continuous improvement across services
o Actively contribute to business planning, risk management, and service delivery innovation
