Community Nursing Role
This role provides comprehensive and coordinated community nursing services to individuals and their families with chronic conditions, recent hospital discharges, or ongoing support needs.
The successful applicant will be responsible for providing clinical care, palliative care, carer support, illness prevention, and health promotion activities.
We are seeking a dedicated and compassionate Registered Nurse who can work in a collaborative team environment, utilizing their skills and experience to deliver high-quality patient care.
Key Responsibilities:
* Provide community nursing services to individuals and their families with complex needs
* Collaborate with healthcare professionals to develop and implement individualized care plans
* Conduct regular assessments and evaluations to ensure optimal patient outcomes
* Develop and maintain relationships with patients, families, and community members to promote health and well-being
Requirements:
* Degree-qualified Registered Nurse with current AHPRA registration
* Proven experience in community nursing or a related field
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
