Job Summary:
The role of the Care Manager Lead involves overseeing a team of direct support assistants and other direct care staff, ensuring their work is conducted in accordance with established guidelines.
Key Responsibilities:
* Supervise Direct Support Assistants and other direct care staff to ensure optimal performance
* Determine staff assignments and reassignments based on program needs
* Manage time and attendance records for accuracy and compliance
* Conduct regular performance evaluations to assess employee growth and improvement
* Develop and implement training programs to enhance service implementation and documentation skills among staff members
* Maintain close observation of staff activities to verify adherence to guidelines
* Collaborate with clinical and program staff to develop individualized habilitation plans tailored to each resident's needs
* Implement or oversee the execution of habilitation plans, ensuring their effective delivery
* Document critical observations and findings for future reference and action
* Oversee household inventories to maintain efficiency and organization
* Ensure continuous supply and equipment availability for seamless operations
* Manage personal finances and related records for individuals under care
* Arrange transportation for recreational, social, work-related, educational, or medical purposes as required
* Respond promptly and effectively to crisis situations and medical emergencies
