As a skilled Home Care Coordinator, you will conduct thorough assessments of client needs, develop tailored care plans, and oversee the delivery of high-quality services that meet the unique requirements of each individual.
Required Skills and Qualifications:
* Relevant qualifications (Cert IV or Diploma in Nursing highly regarded)
* Experience in aged care, NDIS, or mental health case management (preferred)
* Knowledge of My Aged Care, Home Care Packages, and NDIS frameworks
* Proven ability to assess and plan community-based care
* Proficiency in Microsoft Office 365, SharePoint, Outlook, and rostering software
* Excellent communication and interpersonal skills
* Strong organisational and problem-solving abilities
* Collaborative team player with a proactive attitude
