Duties and responsibilities may include:
* Travelling between customer's homes and providing individual and personalised support to customers in accordance with their support plans and/or medical practitioner's directions.
* Working with or under the guidance of Registered Nurses, Allied Health professionals and medical practitioners
* Medication Administration and Complex Clinical Care e. Palliative Care, Diabetes, Bowel Care/ Suppositories, Catheter Care, Stoma Care, Oxygen and more
* Monitoring and communicating on the customer's wellbeing and reporting on progress of customers
* Ensuring that all services are provided in a caring and respectful way in accordance with Just Better Care's policy and procedures, legislative requirements and program specific guidelines (i.e. DVA Community Nursing)
* Provide coaching and guidance to Community Support Professionals and Support Staff/Case Managers as required
* Demonstrating a commitment to best practice and evidence of continued professional development
* Participating in staff meetings and attending training opportunities when required
* Personal care: showering, dressing, grooming and medication assistance
* Domestic services: light household cleaning, laundry and meal preparation
* Social support, social activities, shopping/groceries, travelling to medical appointments
* Supporting customers in the use of aids and personal equipment including: standing or walking frames, wheelchairs, manual handling or electric hoists, sliding sheets or moving boards, hearing aids and other physical aids
* Supporting participants within Just Better Care's Supported Independent Living Home
* Supporting customers with complex and/or challenging needs
* Monitoring the well-being of customers and reporting on their progress
What you will need:
* Minimum Diploma of Nursing
* Current CPR / First Aid Certificate
* Current and satisfactory Criminal Record Check
* Current and satisfactory Federal Police Check, NDIS Workers Screening Card & Blue Card
* AHPRA Registration
* Current Australian Driver's Licence
* Reliable, registered and insured vehicle (comprehensive insurance is preferred)
* Smartphone with internet access
* Proof of of Vaccination (where relevant) i.e. COVID-19 vaccination certificate
* Previous experience in the community care or health/aged care sector
* Understanding of DVA Community Nursing guidelines or previous community nursing experience
* Cert IV in Workplace Training & Assessment
