Case Manager - Country Wellness Connections
An exciting opportunity has arisen for a skilled Case Manager to join our team at Uniting Country SA. As a key member of our Mental Health suite of services, you will be responsible for delivering long or short-term mental health psychosocial or case managed support.

About the Role
The successful candidate will possess excellent communication and interpersonal skills, with the ability to build strong relationships with participants, families, and carers. You will also have a high level of organisational skills, with the ability to manage multiple priorities and deadlines.
You will be required to work collaboratively with a multidisciplinary team, including healthcare professionals, social workers, and support staff. This role requires a high level of autonomy, with the ability to work independently and make sound decisions in complex situations.
We are looking for someone who is passionate about mental health and wellbeing, and who has a strong understanding of the needs of participants with complex needs. You will have a high level of cultural competency, with the ability to work effectively with diverse groups and individuals.
Key Responsibilities
* Support participants to identify their individual recovery needs and develop personal capacity building goals.
* Work collaboratively with multidisciplinary teams to deliver comprehensive care plans and interventions.
* Provide education and support to participants and their families/carers to enhance their knowledge and skills.
* Contribute to the development and implementation of new programs and services that address the evolving needs of our participants.
* Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements
* Current Australian Drivers Licence.
* Knowledge of the Mid North communities and mental health supports and referral pathways.
* Relevant experience in a paid or voluntary capacity.
* A qualification in a Human Services related discipline e.g. Cert IV Community Services or Mental Health.
Applicants currently studying towards a Certificate IV or Tertiary degree will be considered as working towards the essential Certificate IV requirement.
