We are a small, local provider that is growing quickly — and we're looking for a caring and passionate Care Partner who is a registered Nurse to join us and support our clients and team in the Mandurah / Rockingham community.
This is a hybrid role with time spent visiting clients in the community, working from home, and joining our office team. It's a growth opportunity where you'll work alongside the owners and our fabulous home care team, helping to shape the future of our service and bring the Poppy's Difference to life.
About this opportunity
As a Care Partner, you'll partner with older adults and families to ensure they receive safe, high-quality, and truly personalised care. You'll combine professional nursing knowledge with warmth, empathy, and a person-centred approach.
You'll enjoy variety in your week — meeting clients in their homes, communicating via phone and email, actioning requests, completing documentation accurately and planning from your home office and collaborating with the Poppy's team in the office. You'll enjoy problem solving and thinking on your feet in this very fast paced but hugely enjoyable and rewarding role.
Key Responsibilities
Partner with older adults and families to plan, coordinate, and review care goals.
Develop and monitor care plans that reflect individual needs and preferences.
Formalise agreements and ensure clear communication about services and costs.
Maintain accurate documentation and share relevant updates with the care team.
Visit clients in their homes to review progress, provide clinical oversight, and adjust plans.
Balance on-the-road visits, home-based admin, and office collaboration.
Wound care experience is essential.
Incident reporting ensuring client remain safe in own homes.
Completing referrals, liaising with allied health professions.
Supporting the care team in the community, client reviews and budgets.

About you
Minimum qualification: Registered Nurse.
Current AHPRA registration as a Registered Nurse.
Previous experience working with seniors or in aged care.
Minimum of 3+ years of experience working with seniors or in aged care is required.
Knowledge of Home Care Packages and/or community care services (preferred).
Strong communication and relationship-building skills.
Organised, efficient, and able to manage multiple priorities.
Holds a valid Australian Driver's Licence and reliable vehicle.
Permanent or temporary work rights in Australia with no restrictions.
