GPH's mission is to provide equitable heath care with an emphasis on helping vulnerable and at risk of disadvantage populations as well as regional communities.

GPH employs over 400 staff including many qualified health and other professionals including peer workers and community workers. GPH operations cover a large regional geographical footprint in NSW and ACT.

About the position

The Housing and Accommodation Support Initiative (HASI) is a program that supports people who live with a mental health condition to build independence and participate in their community, in a way they choose to. HASI is a recovery-oriented and person-led service, underpinned by recovery principles and values. We strive to walk in partnership with people to find their path to wellness and support them to live a life they value.

The Referral Pathway Coordinator's primary purpose is to promote and manage the HASI program's referral pathway to ensure equitable access for consumers. The Referral Pathway Coordinator will triage referrals, conduct assessments and provide additional support services to referring agencies.

The Referral Pathway Coordinator will work with the Service Manager and referring agencies to refine existing referral pathways to enhance timely access to the service and eliminate barriers to access. This includes screening of referrals, making first contact calls, managing a small caseload and reviewing these functions with the Service Manager. The Referral Pathway Coordinator will also monitor referral pathway KPI's, referral numbers and quality of data collection.

Based across our Ulladulla and Nowra offices, this is a full-time opportunity, fixed term to 30 June 2026. Full time positions have the option to enjoy a Rostered Day Off each month.

We are looking for someone with:

1. Current AHPRA registration as a psychologist or provisionally registered psychologist, mental health nurse or occupational therapist (Mental Health) or accreditation with the AASW as a mental health social worker.
2. Demonstrated skills and experience working in a direct service provision role in a psychosocial support program or mental health service
3. Demonstrated understanding of the operations of the public mental health services and of the legislative frameworks relevant to mental health, including the National Framework for Recovery Oriented Mental Health Services
4. Experience in the development, refinement and implementation of processes and procedures with good attention to detail
