Job Description:
As a dedicated professional, you will be working closely with families, carers and partners to support young people (16–21) as they transition from acute mental health care to community living.
This role involves advocating for young people while ensuring families and carers are informed, supported and included in recovery planning.
You will provide practice leadership by mentoring staff in psychosocial rehabilitation, recovery programs and family-inclusive practice.
Collaborating with colleagues and clinical partners, you will deliver integrated, person-centred care that meets the unique needs of each individual.

Required Skills and Qualifications:
* Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
* Expertise in family engagement work and co-design approaches.
* Experience supporting people with mental health challenges and complex needs, with the ability to coach and develop a team to achieve high-quality service outcomes.
* Hold a current First Aid and CPR certification.
