About The Role

We are looking for Youth Lead Practitioners to support adolescents and young adults aged 9-25 as they navigate life with a disability, from adolescence through to early adulthood.

Key responsibilities include:

* Lead the interdisciplinary youth team that plans a young person's NDIS funded therapy package, acting as the main contact for adolescent and young adult participants and their families
* Utilise assessment and intervention skills to support youth participants to achieve their educational, personal, social, and working life goals
* Work within the social model delivering therapy in diverse settings, from homes and schools to playgrounds, workplaces and community centres
* Hold meaningful, future-focused conversations with participants and their supports
* Empower participants and their families to develop habits and routines
* Build meaningful relationships with participants and their supports (family, carers, friends)
* Document and bill for each session through our online participant management system
* Attend the weekly team meeting in the hub, sharing stories of impact
* Collaborate with Team Leader and Therapy Mentor to tap into clinical knowledge and support
Who We Are Looking For – Qualifications
* Relevant tertiary qualification and registration
* If Youth Worker: Bachelor or Master's degree in Youth Work or Community Services and YWA membership
* If Social Worker: Bachelor or Master's degree in Social Work and AASW membership
* If Specialist Educator: Bachelor or Master's degree in Special Education and AASE membership
* If Registered Nurse: Bachelor of Nursing and AHPRA registration
You Will Also Need – Requirements
* Minimum 12 months experience working with youth aged 9-25
* A positive and flexible approach to work
* Strong communication skills and team player
* Commitment to personal and professional growth
* Demonstrated experience working with people with a disability
* Willingness to work to billable targets
* A valid Australian driver's license
* NDIS Workers Screening Check
* Working with Children's Check
