Clinical Nurse - Community Hospital Interface Program

About The Role
Ripley Specialised Inpatient Services is a state-of-the-art facility that will deliver 90 additional beds in West Moreton as part of Queensland Health's Accelerated Infrastructure Delivery Program. The project will deliver a three-storey facility located on Botany Drive, South Ripley. It is located on the Ripley Health Campus and is co-located with the current Satellite Health Centre. The ward will be operational 24 hrs day 7 days a week and staffed by a multidisciplinary team. The facility is equipped with internal and outdoor therapy areas with an on-demand food service model to compliment therapy programmes.
About You
Advanced clinical knowledge in the area of discharge planning delivering expert nursing care in complex situations while providing support and direction to other team members.
Ability to role model excellent standards of comprehensive, evidence-based nursing care and documentation to facilitate quality patient outcomes, in accordance with National Safety and Quality Health Service and professional standards.
Ability to communicate effectively with members of the healthcare team, patients, relatives and visitors and maintain a positive workplace culture through well developed consultation, liaison, negotiation and conflict resolution skills.
Ability to build and maintain strong relationships and a positive environment within a multidisciplinary team, actively contributing to the learning and development of others to a high standard.
Ability to prioritise clinical and related administrative duties, including effective and efficient planning and organisation of complex caseloads with proven ability to use information and reporting systems appropriate to the position.
