This is a full-time position working Monday to Friday. Your responsibilities will include:
Key Responsibilities
* Conducting regular home visits to assess client needs and wellbeing
* Developing and implementing personalized care plans in consultation with clients and families
* Coordinating services with care organizers and external providers
* Liaising with health professionals, social workers, and community services to ensure integrated client-focused care
* Maintaining accurate records of assessments, care plans, and client interactions
