Overview
Are you passionate about empowering individuals to take control of their care and live independently? We're seeking a dedicated Community Support Worker to provide personalised support within the Consumer Directed Care model at Eyre and Far North LHN.
In this role, you will assist aged, frail, or disabled clients with personal care, medication support, cleaning, meal preparation, transport, and social support. You'll work closely with consumers who actively choose the services they receive, how often, and who provides them—helping them maintain comfort and independence in their own homes.

What We're Looking For
* Completed Certificate III or higher in Individual Support. (Aged Care, Disability or Home and Community)
* Proven experience in exercising own judgment and initiative in the day-to-day execution of a position.
* Experience in the provision of a direct care service in a health-related field.
* Experience in dealing with aged, frail or disabled persons.
* Experience in the use of computer packages e.g. Microsoft Word, Excel.

Checks and Immunisation
* DHS National Disability Insurance Scheme (NDIS) Worker Check
* National Police Certificate (NPC) for employment involving unsupervised contact with vulnerable groups required for this position (E.g. aged care employment or work involving vulnerable clients)
Immunisation Risk for this position is - Category A (direct contact with blood or body substances)
SA Health Services are required to implement the Addressing vaccine preventable disease: Occupational assessment, screening and vaccination policy in the workplace.
