Under supervision, and in accordance with Agency policy, legal, regulatory, and funded program requirements, the Health Care Assistant provides high quality care and support to community and/or facility-based clients and assists with a range of daily living activities that a person would normally do for themselves.

The Team:

The Community Dementia Services is part of Primary Health North. This service provides care for clients in the greater Launceston area seven days a week to deliver excellence in healthcare to achieve improved outcomes for our consumers and the Launceston community.

The Role:

* The Community Dementia team works to improve health outcomes through the provision of safe quality, clinically appropriate nursing care and case management in partnership with patients/clients, their families and other health professionals, supported by the nurse on duty.
* The Community Dementia team is a supportive, engaged and dedicated team enriched by our Hospital Norths 'CARE' values - Compassion, Accountability, Respect and Excellence.

You will need:

* Holds or is working towards a Certificate III in Aged Care or equivalent and demonstrated knowledge and understanding of working with the frail aged, people with disabilities and people affected by dementia.
* Demonstrated experience in providing personal care services and the ability to perform activities of daily living in line with individual client needs in a residential, inpatient and/or community setting.
* Valid drivers licence.

Details of appointment:

* Permanent part time, shift worker position fully rotational, working up to 68 hours per fortnight, commencing as soon as we can onbaord you.

*notwithstanding hours to be negotiated with the successful applicant.

Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.

Salary: $63,110 to $66,049 pro rata per annum. Our Employer 12% superannuation contribution is on top of this amount.

* Salary range is in accordance with Public Sector Unions Wages Agreement 2022
