About the Role
As a Homelessness Case Manager, you'll work directly with individuals and families to create personalised support plans and help them move forward with confidence and dignity. Your days will include safety planning, connecting clients to health and financial services, and linking them to programs such as Towards Independence and Doorways. You'll collaborate with a compassionate team and other agencies to provide wraparound care that makes a lasting impact.
This is a permanent, full-time position based in Berrimah, NT. Flexibility is required; on-call duties and work outside regular hours may be needed.
How You Will Make An Impact
* Encourage and support clients to achieve goals within established case plans, including taking clients to appointments and advocating for services.
* Maintain accurate, up-to-date client records in the TSA electronic database in line with standards and procedures.
* Take appropriate preventative action in potential high-risk situations, per policy and procedure.
* Advocate on behalf of clients with government and non-government agencies, verbally and in writing.
* Establish effective relationships with clients to support self-determination and participation.
What You Will Bring
* Degree qualification preferred (social work, mental health, welfare, or related fields) with relevant experience, or equivalent demonstrated knowledge in the sector.
* Certificate IV in Community Services or related field with corresponding work experience.
* Experience working with people for whom English is not a first language; knowledge of Local Aboriginal Communities and Culture.
* National Police Check prior to commencement.
* NT Working with Children Ochre Card prior to commencement.
* Current First Aid certificate or willingness to obtain.
* Current NT driver's license.
