Care planner - community (adelaide)

**Internal Expression of Interest only**:
**The role**: We are seeking internal expressions of interest for a full time Care Planner Community role based within the northern region. Provide support and enhance the quality of life of our customers with complex requirements, working collaboratively with customer, supporting the co-design and implementation of a goal-orientated support plan that will keep them living at home.
**What you'll do**:

- Develop goal orientated support plans to maximise customer experience, health and quality outcomes
- Provide information, promotion of health services, support, advice, advocacy and coordination of services
- Design and implementations of service options to enhance customer's independence, control, wellbeing and quality of life
- Liaise effectively with external stakeholders, including carers, acute, respite and other community services, medical and allied health professionals
- Provide robust budget management, ensuring that accurate budgets are maintained so the customer can be supported as required

**About You**:
You may be a Care Manager seeking a new challenge or eager to advance your career, along with:

- Ability to assist individual requirements, including customers with a range of complex needs
- A adaptable approach to your work with the ability to prioritise and multi-task
- Empathy, understanding, high degree of integrity and diplomacy
- Excellent interpersonal, communication, negotiation, and problem-solving skills

**You will need**:

- A Diploma in Community Services Coordination, Diploma / Certificate IV in Community Services, or similar equivalent qualification
- OR a Nursing or Allied Health qualification and current registration with AHPRA qualified Enrolled / Registered Nurse or Allied Health Professional
- Experience in the aged sector, preferably in a Care Coordinator / Case Manager role
- Intermediate computer literacy and data entry skills
- Problem solving approach
- To care about supporting older people to live a Good Life
- A driver's license.
