Job Description
The role of a Care and Support Specialist is to provide high-quality care and support services to individuals in need. This involves assessing the needs of clients, developing personalized support plans, and implementing strategies to promote their physical and emotional wellbeing.
Responsibilities
* Conduct thorough assessments of clients' needs and circumstances to identify areas for support and intervention
* Develop and implement customized support plans that cater to the unique needs and preferences of clients
* Provide direct care and support services to clients, including assistance with daily living activities, medication administration, and clinical care
* Collaborate with healthcare professionals, family members, and other stakeholders to ensure comprehensive care and support
* Supervise and coordinate the work of support workers and other team members to ensure effective service delivery
* Maintain accurate records and documentation of client interactions, progress, and outcomes
Required Skills and Qualifications
* Current AHPRA registration as a Registered Nurse or equivalent qualification
* Minimum 3 years' experience working in aged care and disability services
* Demonstrated expertise in clinical assessment, care planning, and case management
* Excellent communication and interpersonal skills
* Ability to work independently and as part of a team
* High level of organizational and time management skills
